What is collected
The contact form collects your name, email address, optional phone number, the reason for your inquiry, and the message you write. A hidden anti-spam field is included and should be left blank.
How it is used
Form submissions are delivered to the practice so your inquiry can be answered. The information is not sold, and it is not used for marketing lists.
How long it is kept
Inquiry messages are retained as needed to respond and, if you become a client, as part of the professional file. You may request deletion of an unanswered inquiry by phone at (972) 521-7800 or through the contact form.
Practitioner-client relationship
Submitting the form or calling does not, by itself, create a practitioner-client relationship. That relationship begins only when both parties agree to the engagement.
